This is to reserve a Date & Time with a $50 Deposit Fee.
This deposit will cover Set up fees and up to 2 Piercings on 1 Client.

Jewelry Cost will be separate and calculated on the Date of Your Scheduled Piercing. Additional Piercings or Separate jewelry Sales will also be available for you.
This is a Deposit for Advance Appointments Only

Parents interested in booking for Minors, Please contact me directly for accommodations.
NO REFUNDS - ReScheduling must be given a 48 hour Notice to prevent forfeiting your Deposit.

*PLEASE UNDERSTAND the time reserved will be set 30-60 minutes apart. And I will do my best to give every collector the Best of my ability and my complete attention. The scheduled Appointment Reservation is an estimated start time. Please arrive at least 15 minutes before appointment to complete a release form. A Digital version can be submitted in advance by Downloading the Lucky Monkey Tattoo Phone App.

Appointments will take place inside of our Ritual Location, 5 Doors down from Lucky Monkey Tattoo. If you need assistance completing paperwork, check in with counter staff at Lucky Monkey Tattoo located at 3024 Packard St Ann Arbor, Michigan.
